If your home has a closed water system, yes, and it is a code requirement. A system is closed if there is a backflow preventer, pressure reducing valve, or check valve on the incoming supply. We verify this on site rather than assuming, because there is no reason to install a tank on a system that does not need one.
Tap the tank. The top should sound hollow and the bottom solid. If it sounds solid all the way up, the bladder has failed and the tank is waterlogged, which means it is doing nothing. A dripping temperature and pressure relief valve on the water heater is the other common sign.
If your system is closed, yes. A water heater replacement is typically when the requirement gets inspected and enforced. We check the expansion tank as part of any water heater replacement rather than treating it as a separate visit.
Pressure spikes every time the water heater heats. That cycling wears out the relief valve, stresses the water heater tank, fatigues fittings and supply lines, and shortens the life of fixtures. The tank costs far less than what it protects.
